Complete the EOP&S Student Petition if you did not meet program requirements outlined in the EOP&S Student Mutual Responsibility Contract (MRC) due to an extenuating circumstance (e.g. COVID-19, flood, fire, illness). Documentation should be provided via this form if applicable. The Fall 2023 EOP&S Student Petition recommended deadline is January 15, 2024 (or at the Director's discretion). Petition decisions will be emailed within ten (10) business days after the petition deadline.
