1. Complete the Initial Intake Form

   Complete our intake form to begin the process of joining Upward Bound

2. Complete the full application and required forms
   
   

   Someone from our office to follow up with you to complete the following:
   1 - Application
   2 - One Letter of Recommendation - use this Teacher Counselor Recommendation form 
   3 - Student Questionnaire (provided by our Staff)

3. Admissions Interview

   After your application is reviewed, you and a parent or legal guardian will be given an Admissions Interview to determine if you are accepted into the program.
