   To meet low-Income criteria, you will need your most current income tax information (Form 1040, 1040-A, or 1040-EZ). If you're classified as dependent, you will also need your parents' income tax information. 
   
   To meet disability criteria, you will need your most current DSPS Accommodation Form.
