This form is used only for students attending Pasadena City College on an F-1 Visa. 

Are you thinking of taking some classes at another SEVIS-approved institution at the same time as studying at PCC? If so, please submit this request.

Employment under Curricular Practical Training (CPT) must be an integral part of an established curriculum and be directly related to the student's field of study. It must be a degree requirement or credit-bearing option. This requirement must be stated in the school's catalog or departmental literature describing the program and must be available to all students in the program. 

Please note: Not all program has CPT requirements.

Is the Program End Date on your I-20 approaching and you still need more time to complete your classes at PCC? If so, please submit this form and the New I-20 Request Form including all required documents.

Do you need an official letter from the ISC? Submit this Letter of Verification Request below. Please also use this form for employment letters. 

You can request a New I-20 for a variety of reasons:

  • Replacement of I-20: (e.g. Lost, Stolen, Damaged)
  • Extension of Program (I-20 Extension)
  • Change of Major
  • Adding Dependents to I-20
  • Regain Legal Status

An F-1 student may request employment authorization based on severe economic hardship caused by unforeseen circumstances beyond the student’s control. These circumstances may include a loss of financial support, substantial fluctuations in the value of currency or exchange rate, or unexpected change in the financial condition of the student’s source of support, medical bills, or other substantial and unexpected experiences.

Do you want to take an online class during Fall and Spring semesters? If you do, make sure you have at least 9 units of on-campus classes first, then log in to PCC Connect and raise a flag for "International Student Online Course Clearance"

If you would like to take online classes during Winter or Summer intersessions, you still need to submit the Online Course Clearance Request. For Winter and Summer intersessions, you do not need to have any on-campus classes.

Optional Practical Training (OPT) is temporary employment that is directly related to an F-1 student's major field of study. At PCC, eligible students can apply to receive up to 12 months of OPT employment authorization after completed their academic studies and obtained an AA/AS degree (post-completion).

Federal Immigration regulations require international students in F-1 status to register for, maintain, and complete a full course load (12 units) in both Fall and Spring semesters. 

Students are allowed to apply for Reduced Course Load (RCL) under only a few specific academic reasons.

This form is for those who are eligible for a Reduced Course Load (RCL) due to severe medical reasons.

Use this form if you would like to release personal information such as Academic Records, F-1 Records, or Admissions letters to a specific individual. 

Each official bank statement must have a sponsor affidavit form attached. Sponsor Affidavit Form indicates who is sponsoring (paying for) you to study at PCC.

Please make sure your sponsor name and the account holder name on your official bank statement is the same. 

If there are multiple account holders on your official bank statement, please submit 1 affidavit form for each account holder.

Are you going to attend another U.S. college/university next semester? Don't forget to transfer your SEVIS I-20 record to your upcoming new school.

In order to transfer your SEVIS I-20 record to the new school:

  • Please complete this Transfer-Out Request Form
  • Submit a copy of your Admission letter from the new school

Are you traveling outside the United States for break/vacation? If so, you must have travel signature on your I-20 (2nd page). In order to get the travel signature, please submit this form with all required documents.
