Dear Faculty,
We are excited to offer both print and online proctoring exam proctoring services during the primary semester! To help us keep accurate records and reduce student wait-time, please
- submit one proctoring request per student at least two business days prior to the scheduled exam date.
- limit exams for students taking the same exam from the same class to five students at any one time.
- drop off any additional resources, materials, or handouts you would like to provide your students at D-300A , indicating the student's name and student ID number.
- ask your students to report to the third floor of the D-Building (D-300A) with enough time to complete their exam before proctoring ends for the day (hours below).
- remind your students to a bring picture ID (e.g., Student ID card, Driver’s License)
- ask your students to bring their own laptops/computers if taking an online exam (due to a limited supply of laptops). For online exams, students work in an online environment while taking the test in the center.
How to Request Proctoring Services
Please follow the instructions and we will contact you as soon as possible.
- Download the Proctoring Application
- Use Adobe Acrobat to complete the Application
- When finished filling in the application, choose File > Save As and rename the file as follows: Student Last Name_Student Lancer ID#_App
To request proctoring services, please upload the following documents as .pdf files:
- Completed Proctoring Application renamed as Student Last Name_Student Lancer ID#_App

When uploading exams, please rename them as Student Last Name_Student Lancer ID#_test title
